It is an additive package aimed at an engine that has worn a little. It is not a repair in a bottle, and it is worth knowing which of those you are buying.

It is an ordinary oil carrying an additive package aimed at an engine that has worn a little: seal conditioners that help hardened seals swell back and regain flexibility, more detergent, and a viscosity that usually sits toward the heavier end of its grade. That last point matters less than it sounds, because a high mileage 5W-30 is still a 5W-30. What it can do is reduce a small weep from tired seals. What it cannot do is fix a failed gasket, a cracked housing or a seal that has torn, and oil arriving on the driveway is a mechanical fault, not an oil choice. Seventy five thousand miles is the figure the industry uses, but behavior is the better trigger: an engine that has begun using a little oil between changes, or a weep at a gasket, or a life of short trips.
A different axis from synthetic against conventional
The first thing to be clear about is that high mileage is not a third option alongside synthetic and conventional. It is a formulation choice that sits on top of either one, so high mileage conventional and high mileage full synthetic both exist and both are ordinary products.

What is in it that is not in ordinary oil
Seal conditioners are the headline. Rubber and elastomer seals harden and shrink over years of heat cycling, and once they have shrunk they stop pressing against the surface they are meant to seal. Conditioners help those seals swell slightly and recover some flexibility, which can close a small weep. It is a real mechanism and it is also a modest one.
Then there is more detergent than a standard formulation carries. An engine with age on it has deposits, and the point of extra detergent is to keep new residue moving with the oil instead of adding to what is already there.
Anti-wear additives are usually present at a higher level, aimed at the parts of an older engine where clearances have opened up and the oil film has more work to do.
And the oil often sits toward the heavier end of its grade. Not a different grade, which would breach the manufacturer's specification, but the thicker end of the permitted band, which helps maintain pressure where wear has made the gaps a little larger.
When the switch is worth making
Seventy five thousand miles is the number the industry settled on, and it is a reasonable place to start thinking rather than a threshold that means anything on its own. Two identical cars at that mileage can be in completely different condition depending on how they were driven.
The signals that actually matter are behavioral. An engine that has begun using a measurable amount of oil between changes, having not done so before. A dampness at a valve cover gasket or a front seal that has not yet become a drip. A vehicle whose life has been short journeys in traffic, which is harder on an engine than the mileage suggests.
Against that, an engine that uses nothing, leaks nothing and has spent its life on longer runs may have no use for the additives at any mileage.
What it cannot do
It cannot restore worn metal. Nothing poured into a sump can. If oil consumption is significant it is usually piston rings or valve stem seals, both of which are mechanical wear, and a heavier oil with more additive may slow the rate slightly while changing nothing about the cause.
It cannot fix a real leak. A conditioner can help a hardened seal recover; it can do nothing for a gasket that has failed or a housing that is cracked. Oil arriving on the driveway is a repair, not a purchase.
And it cannot substitute for the interval. High mileage oil degrades on roughly the same schedule as any other oil of the same base stock. An older engine is generally the last one that should be pushed on timing, because it is already working with less margin than it had when it was new.

Can I use high mileage oil in a newer car?
It will not harm anything provided the grade and any manufacturer approval are correct, but there is nothing for the additives to act on in an engine with tight clearances and fresh seals. You would be paying for a benefit that has no target yet.
Does switching to it commit me to it?
No. Nothing about it is one-way, and moving between high mileage and standard oil at the next change is unremarkable. What is worth avoiding is switching repeatedly in the hope of chasing a symptom that is really a mechanical fault.
